Melissa Warren

Director of Operations

Melissa brings more than 14 years of skilled nursing and post-acute healthcare experience spanning clinical practice, operational leadership, revenue optimization, quality improvement, and multi-facility oversight. She began her career as an Occupational Therapist and advanced through roles including Director of Rehabilitation, Therapy Resource Director, Director of Revenue and Strategic Initiatives, and Director of Operations. She has played an integral role in the acquisition and integration of more than 25 skilled nursing facilities — leading onboarding, operational implementation, employee-retention initiatives, and system integration during periods of organizational growth. That experience is directly relevant to Cornerstone’s Iowa operations, as she understands both the immediate demands of a transition and the longer-term work required to stabilize and strengthen facility performance.

Rachel has more than 17 years of experience in skilled nursing and long-term care finance, revenue cycle management, and acquisition-related transitions. Her background spans business-office operations, accounts receivable, reimbursement, licensing, financial reporting, cash flow management, and operational turnarounds. As Chief Financial Officer, Rachel oversees financial strategy, acquisition integration, revenue cycle, lender and investor relations, and operational performance. She ensures facility and company leadership have the timely, accurate financial information needed to make sound decisions and sustain quality care.

Alan brings more than 15 years of post-acute care and senior living experience, including regional and executive sales and marketing leadership. He helps develop referral relationships and connects patients with facilities that have the clinical capabilities to safely meet their needs — with a focus on responsible census development that supports, rather than compromises, resident care and regulatory compliance.

Cheryl is a respiratory therapist, clinical educator, and healthcare program leader with a master's degree in human biology. Her background spans critical care, pulmonary services, staff education, compliance improvement, and clinical-program implementation. She supports Cornerstone's respiratory programming, clinical education, employee competency, and quality improvement efforts.

Apollonia brings extensive skilled nursing experience in clinical leadership, nursing education, MDS, and resident assessment. She holds a Master of Science in Nursing Education, a Bachelor of Science in Nursing, and RAC-CT certification. In a consulting capacity, she supports MDS accuracy, clinical documentation, reimbursement coordination, and staff education.

Trisha is expected to join Cornerstone following the September 1 acquisition. She will provide additional clinical support to nursing facility leadership, including assistance with regulatory readiness, clinical-system implementation, staff education, and quality-of-care initiatives. Her addition will further strengthen the clinical resources available to facility administrators and Directors of Nursing.
