Melissa Warren

Director of Operations

Melissa brings more than 14 years of skilled nursing and post-acute healthcare experience, combining a clinical background with operational leadership, revenue optimization, quality improvement, regulatory support, and multi-facility oversight. She began her career as an Occupational Therapist and progressed through leadership roles including Director of Rehabilitation, Therapy Resource Director, Director of Revenue and Strategic Initiatives, and Director of Operations. Melissa has worked directly with facility leaders to improve clinical outcomes, strengthen regulatory compliance, optimize reimbursement, support survey readiness, and implement systems that improve both resident care and financial performance. She has led interdisciplinary teams, supported Medicare compliance and managed care initiatives, and assisted with the development of specialized clinical programs.  Melissa has also played an integral role in the aquisition and integration of more that 25 skilled nursing facilities. Her responsabilities included facility onboarding, operational implimentation, emplyee-retention initiatives, and system intergration during periods of orginizational growth. This experience is directly relevent to Cornerstone's Iowa aquistions because she understands both the immidiate demands of a transition and the longer-term work neccesary to stabilize facility operations.

Rachel Silver

Chief Financial Officer

Rachel has more than 17 years of experience in skilled nursing and long-term care operations, revenue cycle management, healthcare finance, and regulatory transitions. Throughout her career, she has held progressively responsible leadership positions overseeing business-office operations, accounts receivable, collections, financial systems, licensing, reimbursement, and acquisition-related transitions. Rachel's experience includes facility acquisitions, operational turnarounds, healthcare reimbursement, regulatory compliance, financial reporting, cash flow management, and revenue cycle improvement. Because her background encompasses both facility operations and finance, she understands the relationship among resident census, reimbursement, expense management, working capital, regulatory requirements, and the resources necessary to support quality resident care. As Chief Financial Officer, Rachel is responsible for financial strategy, acquisition integration, lender and investor relations, financial reporting, revenue cycle oversight, cash management, reimbursement, and operational performance initiatives. She establishes and monitors the financial controls necessary to support stable operations and ensures that facility and company leadership have timely, accurate information with which to make decisions.

Alan Hoskins

Patient Care Liaison Resource

Alan has more than 15 years of experience in post-acute care and senior living, including regional and executive sales and marketing leadership. His experience includes responsible census development, hospital and physician relationships, customer-support improvement, reputation recovery, specialty clinical-program development, and training for facility and clinical leaders. Alan has led regional strategy across 20 post-acute centers and has experience working with hospital systems, physicians, managed-care organizations, and facility leadership teams. As a Patient Care Liaison, he helps develop referral relationships and connects appropriate patients with facilities that have the clinical capabilities and resources necessary to safely meet their needs. Cornerstone's objective is not simply to increase census. Referrals must be evaluated against each facility's staffing, clinical competencies, equipment, environment, and ability to meet a prospective resident's needs. Responsible census development must support, rather than compromise, resident care and regulatory compliance.

Cheryl Schomp

Clinical Resource

Cheryl is an experienced respiratory therapist, clinical educator, and healthcare program leader with a master's degree in human biology and formal education in respiratory therapy. Her background includes critical care, pulmonary services, staff education, policy and procedure development, compliance improvement, budgeting, clinical-program implementation, and management of clinical personnel. In a recent program-leadership position, Cheryl increased departmental compliance from 31% to 85%, expanded pulmonary services, assisted with opening respiratory programs in four buildings, and developed clinical policies, competencies, and staff-education initiatives. Cheryl's experience will support Cornerstone's plans to enhance respiratory programming, clinical education, employee competency, quality improvement, and resident-centered services.

Apollonia Williams, RN, MSN, RAC-CT

Clinical and MDS Consultant

Apollonia has extensive skilled nursing experience in clinical leadership, nursing education, MDS, reimbursement, and resident assessment. Her prior positions include Director of Nursing, Assistant Director of Nursing, Unit Manager, MDS Coordinator, Director of Education, and Multi-Facility Clinical and MDS Resource. Apollonia holds a Master of Science in Nursing Education, a Bachelor of Science in Nursing, and RAC-CT certification. In a consulting capacity, she supports MDS accuracy, clinical documentation, resident-assessment processes, reimbursement coordination, staff education, and the development of sustainable clinical systems.

Trisha Haubrich

Clinical Resource

Trisha is expected to join Cornerstone following the September 1 acquisition. She will provide additional clinical support to nursing facility leadership, including assistance with regulatory readiness, clinical-system implementation, staff education, and quality-of-care initiatives. Her addition will further strengthen the clinical resources available to facility administrators and Directors of Nursing.
